This is a grant funded position to build community-based prevention and educational services in two small communities. The Prevention Program Coordinator (PCC) will be responsible for implementing best practice alcohol/drug, anti-bullying and positive youth development programming in partnership with the local school district. Program development, youth group work, and facilitating workshops for parents and other community members will be ongoing.  The PCC will also spearhead the HK Community Coalition, a group of interested partners working together to support the mental health and well of the HK community.


Essential Duties and Responsibilities
The following duties are normal for this position. This list is not all-inclusive because our work is ever changing and never boring. Updated duties may be required and assigned

Prevention Education Services and Responsibilities:

  • Conducts school-based groups or after-school clubs with a focus on health and wellness topics (relationships, stress, positive developmental assets, self-esteem, bullying)
  • Works with the Executive Director and Program Evaluator to conducts community surveys and share the information with community partners. Collaborates with health specialists and civic groups to ascertain health needs, develop desirable health goals, and determine availability of professional health services with a focus on positive youth development and family health and wellness
  • Develops and maintains cooperation between public, civic, professional, and voluntary agencies. Works to identify community partners and supporters and meets regularly with this group over the course of the year.
  • Attends and participates in training/informational sessions that are appropriate for Health Educators.
  • Maintains a calendar of events of educational and outreach activities and working with the Office Manager keeps that calendar current.
  • Provides visits to school health programs for on-site education for faculty, youth or paraprofessional as issues arise
  • Works with the Co-Chairs of the Haddam-Killingworth Healthy Community Healthy Kids (HCHK) Coalition to execute coalition strategies to advance community wellness initiatives
  • Active participation and/or staffing committees of the HCHK Coalition as appropriate
  • Track data and oversee State and grant reporting of activities

Administrative Responsibilities:

  • Represents the agency at local, regional and statewide meetings and keep abreast of professional and community resources
  • Attends and actively participates (when appropriate) on community, regional and State committees to provide integrated services to the youth and families of HK
  • At the request of the Executive Director, attends Board meetings in order to provide overall updates on the department, clinical programs and revenue.
  • Performs other related functions as assigned or required

 Preferred Experience and Credentials:

  • Computer skills using Microsoft Office
  • Experience working with small groups or in a school setting
  • Experience working in the field of health promotion, disease and/or substance use prevention
  • Demonstrated ability to be flexible and open to new ideas is paramount

Terms and Benefits:

  • Position is full time, salaried, with a flexible 40- hour (M-F) work week.
  • Three weeks paid vacation; 8 sick/personal days. 3 of which are available from the date of hire)

  • Mileage reimbursement
  • A limited monthly stipend will be paid toward the cost of the individual or group health and dental insurance offered by the agency

This is a full-time grant funded position.  The grant runs through June of 2024 with the potential for renewal/extension based on the success of the program.

Job Type: Full-time

Pay: $50,000 - $58,000 per year



Requirements

Minimum Training and Experience Required:
· A Bachelor’s Degree in Public Health, Community Organizing, Health Education or Social Work

  • Possession of a valid State of Connecticut driver’s license for travel between offices, schools and meeting attendance
  • Clearance of a required background check
