Continuum of Care, Inc. is a not for profit organization operating licensed group homes, supported living programs, intensive 24–hour supported living programs, community–based crisis and respite services, and residential case management services. The Agency touches the lives of more than 1,200 individuals annually in various locations throughout Connecticut.
With a primary mission of assisting individuals with serious mental illnesses and substance abuse to live safe and independent lives in their community, Continuum of Care’s mental health division operates over 30 diverse shared housing, and scattered site apartment programs. We have transitioned dozens of individuals from institutional settings into community based homes and service hundreds per year with our growing crisis stabilization services.
Continuum needs passionate, enthusiastic, and motivated employees to join our growing team. Staff work in conjunction with clients to provide case management or supervision, or work in our specialized crisis stabilization programs, sober housing, hospital step-down services, and nursing home diversion programs. Company History
Continuum of Care established its roots in the Greater New Haven area in 1966 with the development of The New Haven Halfway House, its first residential program for persons with mental illness. Those grass roots laid the foundation for the development of a comprehensive array of residential service options during the ensuing 42 years for persons with mental illness as well as individuals with developmental disabilities.
